Mandatory Occurrence Reporting

Certain aviation occurrences must be reported by the persons or organizations identified in the applicable requirements. A report supports regulatory safety oversight and may be required in addition to other operational or investigation notifications.

Before submitting

• Confirm whether the event meets a mandatory-reporting requirement.
• Gather the known factual information without delaying the initial report.
• Use the current approved form or secure reporting channel.
• Retain supporting records according to the applicable procedure.

If the event is an accident or serious incident, also follow the urgent AAIU notification pathway.

Staging content: operational requirements, contacts, fees, processing information and publication links must be confirmed by the responsible Department owner before public release.
